How to use this form
This form captures contact details and optional CUF Newsletter Consent and can add respondents to a Salesforce Campaign. 

Make a copy of this form for each new Signup. 

To grant respondents access to specific resources, configure the form Notifications.  Provide a link in the acknowledgment text or a redirect url to a page on your website. 

To send your respondents a confirmation email configure the form Notifications.  Enable the Auto-Responder and specify details of the email to be sent.

To add form respondents to a Salesforce campaign, provide the Salesforce Campaign ID as the default value in the field below: 

Provide the default value:  Select this field, click the blue 'Options' button.  Find Default Value in the Field properties (menu appears on the left of the form). Enter your Salesforce Campaign ID as the Default Value.  ID format: 701Q5000003JaUIIA0
If you want respondents to be added to Mailchimp for a further email journey, please contact your Salesforce/Mailchimp Administrator who will link your campaign members to Mailchimp and help you set up the journey.
